Idaho State law requires every student to have a complete immunization record to attend public school. The following immunizations are required:
- 5 DTaP
- 2 MMR
- 3 Hepatitis B
- 3 Polio
It’s Tuesday, April 20 from 8 – 8. A parent or legal guardian must submit one of the following as proof of legal residential address:
- Land line telephone bill
- Gas bill
- Electricity bill
- Water bill
- Hook up receipt to any of the above
- Rental/lease/purchase agreement
The bill must show
· Recent date, within 30 days
· Parent or legal guardian’s name
· Address
If you are just living with someone, we have a second party form in the office that would need to be notarized. |

Parents and Students,
We have new Centennial t-shirts to sell this year. The shirts are $10.00, and are available in Youth Medium, Large, X-Large, Adult Small, Medium, Large, and XL. Additional sizes can be ordered but may carry a small fee. If you would like to get a shirt, please send $10.00 with your student and their teacher will get it for them. Or, they will be available during conferences or school programs. If you have any questions please call the school (468-4627).

If are new to Centennial, or have left and are returning (after an absense) please bring the following items with you when you register:
1. Child's Birthcertificate
2. Child's Immunization Records
3. Utility Bill (water, gas, electric, home phone) dated within the last 30 days.
If you are a current Centennial Student you DO NOT need to reregister. We will be sending a packet of papers home on the first day for you to fill out with all your updated information.
If you have any questions or concerns, please feel free to call the school starting August 12th at 8:00. |
